Pay Range: $16 - $17.55/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

• Provide supervision of individuals in accordance with their Person-Centered Support Plan.
• Administer Medication as needed.
• Provide transportation to consumers.
• Support consumers in activities of daily living such as meal prep, toileting, personal hygiene, as appropriate.
• Assist in Community Integration Activities and access to community Supports including medical appointments, social security access, etc.
• Administrative:Complete required documentation in a timely manner.
• All other duties as assigned by supervisor.

Key Requirements and Technology Experience:

• Skills- Medication, Vital, Caregiving.
• High School diploma or GED is required, associates or bachelor’s degree is preferred
• Valid, non-restrictive Driver's License.
• CPR Certification Card/First Aid Certification.
