Social Worker / Community Educator AND/OR Outbound Sales Rep /Community Educator Interim HealthCare Utah is growing again and looking to expand the care management team. Interim HealthCare Utah, the leading home care provider in Utah, is hiring a Social Worker / Community Educator for Salt Lake County. Compensation is a base salary plus residual commission for the entire time clients are on service--often several years. You might be working in the health care sector as a social worker or nurse and looking to expand your horizons. We have a fantastic tool box of resources that make educating the community fun and easy. Qualifications:

• LMSW, LBSW, preferred with experience in the senior care industry
• Occupational Therapist, Physical Therapist, LPN, RN considered
• Exceptional presentation skills and highly organized
• Experience with PowerPoint
• Experience with the elderly preferred
• Passion for the elderly a must

Primary Responsibilities (including, but not limited to):

• Identify all local influence centers (hospitals, nursing homes, assisted living with rehabilitation or respite care, doctor's offices and other health providers) to determine lead sources.
• Build and maintain database of potential referrers.
• Conduct training sessions to professionals and families that focus on dementia care and Parkinson’s disease care and other senior challenges and risks.
• Personally visit and arrange meetings with persons responsible for or in a position to refer clients; this includes private, public, and non-profit organizations.
• Arrange presentations of Interim HealthCare Utah services at staff meetings in hospitals, nursing homes, doctor’s offices, and private companies as well as brokerage companies looking for labor hire partners.
• Attend trade shows, conferences and networking events representing Interim HealthCare Utah services. Network with others in the industry to develop additional lead sources.
• Manage periodic on-call calls with internal staff.
• Maintain and manage a (CRM) Customer Relationship Management System to track leads and follow-up on leads.
• Achieve weekly visit frequency with key targets and deliver messaging as per the 52 Week Marketing program.
• Use tracking sheets to record activity and submit to manager weekly.
• Meet with manager weekly to discuss opportunities, referrals status, activity, etc.
