Job Description

**Coverage Dates Needed for Urology Services (Ongoing Needs Through 2026 TBD):**

- 

**Friday, October 24 at 5 AM 

– Monday, November 3 at 9 AM** - 

**Monday, November 10 at 7 AM – Friday, November 14 at 5 PM** -

**Monday, November 24 at 7 AM – Thursday, November 27 at 5 PM** -

**Monday, December 1 at 12 PM – Friday, December 5 at 5 PM** - 

**Monday, December 8 at 7 AM – Friday, December 19 at 5 PM** -

**Monday, December 22 at 7 AM – Friday,

December 26 at 5 PM**

**Service Expectations:**

- Coverage will include both clinic and call responsibilities, with clinic hours running from 7 AM to 5 PM daily and night call from 5 PM to 7 AM.

- The preference is for a provider to cover both clinic and call during the weekdays. However, the client is also open to providers who can take 24-hour call without clinic responsibilities during the specified dates.

**Long-Term Assignment Potential:** While starting with sporadic dates, there is an ultimate goal to find a provider who can offer consistent coverage for a longer-term assignment.

**Patient Population:** - Focus is on adult patients.

- Procedure volume is approximately 2-5 patients per day.

**Technology Used:**

- Electronic Medical Record (EMR) system: EPIC

**Compensation:**

- Full travel expenses will be covered.

**Qualifications Required:**

- Board Certified or Board Eligible within five years of residency (Required)

- Indiana medical license at the time of submission or IMLC (Required)

- CDS Certification and DEA Registration (Required)

- ACLS certification (Required)

- Robotics training (Highly Preferred; please specify if applicable at the time of submission)

**Certification Requirements:**

- ACLS - Board Certification

**State License Requirements:** - Indiana license is mandatory.
