Judge Healthcare is seeking a Nurse Practitioner in Lee County, FL!

Summary : The Nurse Practitioner will deliver comprehensive healthcare to patients across various care settings, including homes and community-based facilities. This role involves conducting assessments, managing chronic conditions, prescribing medications, and promoting preventive care. The NP will collaborate with a multidisciplinary team to ensure quality, coordinated, and personalized care.

Key Responsibilities :

• Conduct in-home and facility-based patient visits , aiming for 25+ weekly.
• Perform comprehensive assessments : health status, risk analysis, and physical exams.
• Diagnose and treat using evidence-based practices.
• Manage chronic diseases and educate patients for optimal health outcomes.
• Prescribe and adjust medications; provide medication education .
• Emphasize preventive care : immunizations, lifestyle improvements, screenings.
• Collaborate with physicians and health teams to ensure seamless patient transitions.
• Maintain accurate electronic medical records (EMR) within 48 hours of visits.
• Participate in after-hours, on-call care as needed.

Qualifications :

• Licensure : Active, unrestricted NP license in the practicing state.
• Certified as a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner .
• Autonomous provider status eligibility (preferred two years of primary care experience).
• Current DEA and BLS certifications.
• Strong diagnostic, clinical, and communication skills .
• EMR proficiency , valid driver's license, and reliable transport with insurance.

Preferred Experience :

• Knowledge of HEDIS/STAR measures, risk adjustment, and HCC documentation.
• Prior home-based visit experience.

Physical Requirements :

• Ability to lift up to 20 lbs., remain mobile, and navigate diverse settings.
• Mental focus on problem-solving, decision-making, and data interpretation.
